How to Transfer Care to Village

Welcome to Village Pediatrics!

ONCE WE HAVE ACCEPTED YOUR FAMILY INTO THE PRACTICE (please click HERE for a Transfer Patient Application), follow these steps to get your child(ren) enrolled:

  • Please review our vaccine policy and financial policy.
  • Here you will find our new patient forms. These can be completed and securely submitted via the website to save time at your first visit.
  • Complete a medical record release form and submit it to your previous pediatrician’s office. This is especially important for children with any significant past medical problems, including but not limited to ADHD, asthma, behavioral or psychiatric concerns, growth delays, any specialist visits, abnormal labs, etc.
  • Call your insurance company to verify that our office is listed, and to change your child’s Primary Care Provider (PCP) to her if necessary. See our list of contracted insurance plans; if your plan is not on that list, we are always happy to see you out-of-network or on a cash-pay basis.
  • If moving from out of state, please fax our office your child’s immunization records as soon as you can. Please note: we will require this prior to scheduling a well-child check up.
  • Contact the office to schedule your first visit with us!


Is your child sick?

We are happy to see them as a new patient even for same-day sick visits, but please do not come to walk in time for your first visit- we like to have some extra time to get acquainted with you and your child.

Check-up time?

Schedule your child’s next well-child care visit with us per our care guidelines. Please note: insurance plans differ as to whether they will cover a check-up for kids older than three years every calendar year or every 365 days. It’s up to you to know when your child is due for a check-up and when your insurance will cover it.

Our checkup schedule: 

  • newborn
  • 6 month
  • 21 month
  • 2 weeks
  • 9 month
  • 2 year
  • 1 month
  • 12 month
  • 2.5 year
  • 2 month
  • 15 month
  • 3 year
  • 4 month
  • 18 month
  • annually

We offer option 3m and 5m checkups for parents with extra questions/concerns, but these may not be covered by insurance.

Need to follow-up for chronic conditions?

If your child needs management for a long-term medical condition, please schedule an appointment separate from any check-up or other sick visit.

Examples of chronic conditions include:
  • acne
  • ADHD
  • allergies
  • asthma
  • constipation
  • depression
  • irregular menstrual cycles
  • eczema
  • anything that requires medications on a regular basis
